Job Summary
We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to provide exceptional in-home care to individuals with diverse needs. In this vital role, you will assist clients with daily activities, monitor their health status, and ensure their comfort and safety. Your expertise will help promote independence and improve quality of life for those receiving home health services. Duties
Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using medical terminology
Support patients with medication administration following prescribed guidelines while maintaining HIPAA compliance for patient confidentiality
Perform patient observation to identify changes in health status or behavior, especially in individuals with dementia or Alzheimer’s disease
Provide specialized care for memory care, hospice, and long-term care patients, including catheter care and hospice support
Operate medical equipment such as Hoyer lifts, EHR (Electronic Health Record) systems, and EMR (Electronic Medical Record) systems for accurate documentation
Assist with household tasks like cleaning, laundry, meal preparation, and ensuring a safe environment for clients with disabilities or in assisted living settings
Support patients during transfers using Hoyer lifts or other mobility aids while ensuring safety protocols are followed
Document medical records diligently and communicate effectively with healthcare teams to ensure continuity of care
Requirements
Valid certification as a Home Health Aide from an accredited program
Proven experience working in home health, nursing homes, assisted living facilities, or long-term care environments
Knowledge of medical terminology and procedures related to patient care and medication administration
Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice or dementia care are a plus
Experience working with individuals with developmental disabilities or cognitive impairments like Alzheimer’s disease or dementia
Ability to work independently while following detailed care plans and medical documentation protocols
Strong communication skills to collaborate effectively with clients, families, and healthcare professionals
Flexibility to work live-in shifts or community-based assignments as needed
