Duties
Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor and observe patient conditions, noting changes in health status and reporting to healthcare professionals promptly
Support medication administration and manage medical documentation using Electronic Health Record (EHR) systems while maintaining HIPAA compliance to protect patient confidentiality
Perform vital signs assessments such as blood pressure, pulse, temperature, and respiration rates to track health trends
Provide specialized care such as catheter care, dementia care, Alzheimer's and memory care, hospice support, and end-of-life assistance
Help with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for patients
Use patient transfer methods safely with equipment like Hoyer lifts or other assistive devices while adhering to home health regulations and safety protocols
Requirements
Proven experience working with individuals with disabilities or in assisted living, nursing home settings, or home healthcare environments
Knowledge of medical terminology, medication management, and medical documentation procedures
Certification in first aid and CPR; basic life support (BLS) certification preferred
Ability to work with geriatric patients and those with developmental disabilities or dementia-related conditions
Familiarity with home health agency operations, EMR/EHR systems, patient transfer methods, and HIPAA regulations
Physical ability to perform heavy lifting when necessary and assist with patient mobility using appropriate equipment like Hoyer lifts or transfer belts
Excellent communication skills for client interaction and collaboration with healthcare teams

Pay: From $18.00 per hour
