As a Certified Home Health Aide, you’ll contribute to our success in the following ways:

  • Provide personal care support, including bathing, grooming, dressing, and hygiene assistance

  • Assist clients with mobility, transfers, and daily living activities

  • Prepare meals and assist with feeding when needed

  • Perform housekeeping, laundry, and household tasks

  • Provide companionship and emotional support to improve quality of life

  • Assist with errands, grocery shopping, and transportation to appointments (when applicable)

  • Promote a safe, comfortable, and respectful environment for each client


What is Required?

  • New Jersey Board of Nursing Certified Home Health Aide CHHA License required

  • High School Diploma or GED preferred

  • Experience providing care to elderly or disabled individuals preferred (professional or personal experience)

  • Compassionate, patient, and dependable personality

  • Strong communication and interpersonal skills

  • Reliable transportation to travel to client homes

  • Ability to lift, stand, bend, and assist clients as needed (up to 50 pounds)

Employment Type: Full-Time / Part-Time / Per-DIem (Flexible Scheduling Available)Compensation: Competitive Pay ($17.00 – $19.00/hour) + Weekly Pay + Paid Orientation


What You Will Do:

  • Assist clients with personal care needs, including bathing, dressing, grooming, oral care, feeding, and medication reminders

  • Support mobility and daily living activities, including walking, transfers, repositioning, and toileting

  • Prepare meals and assist with light housekeeping, including laundry and maintaining a clean, safe environment

  • Provide transportation and support for errands and appointments as needed

  • Offer companionship and emotional support, fostering a respectful and engaging environment for clients

  • Observe and report changes in client condition to the care team and document care accurately

  • Follow care plans and safety protocols to ensure high-quality, client-centered care


What You Will Have:

  • Active New Jersey Certified Home Health Aide (CHHA) license

  • Current TB screening/QuantiFERON or Chest X-Ray (within 1 year)

  • Proof of U.S. work authorization (valid, unexpired documents)

  • Reliable transportation

  • Strong communication and interpersonal skills

What You'll Do

As a CHHA with Grace Healthcare, you'll provide direct care that makes a meaningful impact, including:

  • Personal Care: Assistance with bathing, grooming, dressing, skincare, and mobility

  • Meal Preparation: Creating simple, nutritious meals according to the patient's care plan

  • Home Management: Ensuring a safe, clean, and comfortable home environment

  • Documentation: Recording care and activities using CellTrak

  • Team Collaboration: Reporting condition changes to RN Case Managers for prompt action


What We're Looking For

  • Valid NJ CHHA license in good standing

  • Clear communication skills in English (reading, writing, and speaking)

  • Ability to lift, push, or pull up to 50 lbs

  • Dependable transportation and willingness to travel within Middlesex,Union, & Somerset Counties
