Job Summary

We are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.


Responsibilities

  • Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles

  • Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care

  • Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor

  • Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert

  • Give medications and immunizations

  • Engage patients in exercises or other activities


Schedule

Monday 12pm to 5pm

Wednesday 1pm to 6pm

Friday 1pm to 6pm


Rate is $18 per hour

What You’ll Do as a Certified Home Health Aide:

  • Providing hands-on assistance with activities of daily living, including bathing, dressing, grooming, toileting, and mobility, and assisting with meal preparation, feeding and hydration.

  • Monitoring basic health status and reporting changes to the Community Director.

  • Offering companionship, emotional support, and reassurance to the residents, and leading or assisting with engaging activities and social interactions.

  • Performing light housekeeping and laundry to maintain a safe, comfortable home.

  • Safely assisting with resident transfers and mobility as needed.

  • Supporting a safe, nurturing environment that meets residents’ physical and emotional needs.

What You'll Need:

  • Current New Jersey Certified Home Health Aide (CHHA) license required

  • Certified Dementia Practitioner (CDP) preferred, but not required

  • Previous experience in memory care, assisted living, or senior care preferred, but not required

  • Must be at least 18 years of age or older

  • Ability to lift and support up to 75 lbs for resident transfers and mobility assistance

Position Summary

We are seeking a skilled and compassionate Phlebotomist to support our nursing team in the collection and processing of blood and urine samples, ensuring high standards of clinical safety and patient comfort. This role is integral to supporting the medical needs of our patients and maintaining compliance with treatment protocols.

Key Responsibilities

  • Perform venipuncture and specimen collection for laboratory testing.

  • Accurately label, process, and prepare specimens for transportation or in-house analysis.

  • Maintain cleanliness and infection control standards in collection areas.

  • Assist nurses in monitoring patients during blood draws and flagging any adverse reactions.

  • Document all procedures and patient interactions in electronic health records (EHR).

  • Communicate effectively with the clinical/nursing team regarding lab needs and patient concerns.

  • Maintain an organized supply inventory and restock phlebotomy materials as needed.

  • Follow all HIPAA and OSHA regulations and maintain strict patient confidentiality.

Requirements

Qualifications

  • High school diploma or equivalent required.

  • Completion of an accredited phlebotomy training program.

  • Certified Phlebotomy Technician (CPT) preferred.

  • Minimum of 1 year of experience in a clinical or behavioral health setting is highly desirable.

  • CPR certification required or must be obtained within 30 days of hire.

  • Strong interpersonal and communication skills with a patient-first attitude.

Working Conditions

  • Clinical setting within a behavioral health treatment facility.

  • Occasional lifting or assisting patients may be required.

  • Work is primarily during standard hours but may include occasional evenings/weekends.
